Kids’ Church Parent Agreement Form

Please review and check the following boxes stating you have read the statements below:

#1 *

All volunteers are background checked, and each class has at least two teachers.

#2 *

We ask you not to send your child if they have had a fever (over 100) in the last 24 hours, have vomited in the last 24 hours, have a runny nose or sore throat, diarrhea in the last 24 hours, or any other contagious sickness. Please notify us if you find out your child has a contagious sickness (excluding the common cold) after they have been in Kids’ Church.

If a child needs help using the bathroom, we will get you out of service to help them. You may want to take your child to the bathroom before dropping them off at Kids’ Church.

#4 *

If a child is having a very difficult time, such as crying without stopping, or unable to follow class rules, we will keep you in the loop and come to inform you during service!

Wait until check-in time to start checking your child in! :)

We serve pretzels, Cheerios, or animal crackers (fruit loops in Biggs room). Please let us know of any allergies. We may give out candy, but we ask kids to eat it AFTER they leave Kids’ Church.

I agree to let my child be photographed for use on the church website, walls, or social media.
